This site is designed to hlep you fill int he mission forms that are most used by our flotilla members, and to expedite the process by clicking on the links at the left hand side of this page.

My job is to collect and supervise matters pertaining to the collectino, recording, and forwarding of the flotilla auxdata information, and keep members informed of all developments in this area.

AUXDATA is an interactive real-time electronic data processing system.  It is designed to store all qualifications and activities of each auxiliary member in a computer data dase.  This information is available to authorized users on-screen and in hard copy as management reports that select and summarize pieces of this information.

Who should complete the forms

All active CG Auxiliary members should regularly record their Auxiliary activities, and send reports to the Flotilla FSO-IS officer on a regular and timely basis.

Some forms "e-forms" may be filled out on-line and sent to the FSO-IS by email utilizing the links to the left. Others, including the important Change of Member Information (7028) are not yet available as e-forms. All forms can be accessed at the National CGAUX website.

The following persons in our Flotilla have the responsibility for writing and sending an appropriate form to the FSO-IS. (by email)

  • Each individual member should fill out the MEMBER ACTIVITY LOG (form 7029), and send it to the FSO-IS. (email)
  • The individual in charge of a group mission should fill out the ACTIVITY REPORT-MISSION form 7030.  All participating members are to be listed by the person in charge.  An example would be in Public Instruction..  The lead instructor should fill out the 7030 form for his/her evening of instruction, including every member who assisted in the activity for the evening, and the form sent to the FSO-IS. (email)      For Public Affairs (PA) missions, the person in charge of the booth should fill out the 7030 form for all involved, and send the form to the FSO-IS. (email) A member doing a Public Affairs activity singularly, should fill in a 7030 form and send it to the FSO-IS Officer. (email)
  • Each Vessel Inspector should fill out his/her own activity form (7038) and email it to the FSO-IS . (email)

All forms should be completed as soon as possible after a mission or an event has taken place.  The District requests no longer than 60 days maximum.

How the forms get processed

The following forms follow the IS flow of forms:

  • Annual Currency Maintenance Task form (CG-05-07) attached to the Patrol 7030 form
  • Change of member information. (CGAUX-33/ANSC 7038)
  • Public Education completion (part of the final 7030 mission report)
  • Activity Report -Mission (CGAUX-26/ANSC 7030)
  • SAR Incident Report (CG-4612-AUX/ANSC 7034)
  • Workshop Attendance Report (CGAUX-31/ANSC 7029)
  • Member Activity Report(CGAUX-31/ANSC 7029)
  • Aids to Navigation Report (CG-5474/ANSC 7054) Multiple routing path
  • Activity Report Vessel Examinations (ANSC-7038)
  • Activity Report RBS (Dealer) Visitation (ANSC-7046)

Forms that are routed through other Staff Officers:

  • Vessel Facility Inspection & Offer for use forms (ANSC) 7003)
  • NOAA cooperative chart up dating program (NOAA form 77-5(7-92)/ANSC 7037

FSO-IS Action:  Reviews the forms received from members to verify that they are correctly filled out and legible. Forms that are unclear or that contain errors that cannot be corrected by the FSO-IS are returned to the member. On the 1st and 15th of the month the FSO-IS mails all AUXDATA forms to the SO-IS. 

SO-IS Action: Unclear forms are returned to the FSO-IS Officer.  Enters data from all forms received into the AUXDATA computer system.  Returns forms to the appropriate FSO-IS after data is entered. SO-IS stamps the date entered on each form. 

FSO-IS checks the returned forms against the Flotilla posting and corrects any errors.
